NURS 628 - Managing Community Health Issues and Systems

SP. This course will study the specific clinical nurse leader roles and responsibilities for providing clinical leadership for patient care practices and delivery, including the design, coordination, and evaluation of care for individuals, families, groups, and populations within the community/population health specialty. The focus is for students to integrate patient data and apply evidenced-based strategies for managing acute and chronic illnesses for the best patient outcomes in population health. Students are expected to participate in identification and collection of care outcomes and be accountable for evaluation and improvement of patients at the point of care. This includes risk anticipation, lateral integration of care for patients, team leadership and collaboration with other health professionals, use of information systems and technologies, and stewardship and leveraging of resources to advocate for patient in population health and community settings for point of care. Prerequisite(s): NURS 607.
